I'm not a doctor, but I've heard of vaginismus and I think it can be like that. There are real doctors who can help with this kind of thing, she could share about this experience with her primary care and that doctor should be able to refer to someone.
But there's a whole hell of a lot more to sex than just sticking the penis into things, make sure she's having a good time, for a person's body to betray them in such an intimate moment can be terribly crushing. Before we worry about orgasms, make sure she knows you care about her enjoying the experience.
