Riding a road bike is best for me. I've been on nearly every painkiller. Actually prescription stimulants to focus past the pain and a regular physical therapy routine are my 10+ year empirical best scenario. Road bikes have a different level of tailored fit that makes much more possible when people have major issues. My thoracic spine and neck are damaged. The thoracic is the area along the ribcage. It is super rare to damage the thoracic and unlike anything people associate with back problems normally. I can neutralize that part of my back with my bike fit and adjusted to me. It seems impossible that I could ride given my condition, but I manage. I'd be in far worse shape otherwise.
