I know arlong was worse, but i think Worst in the East might be about Don Krieg, known as the ruler of the east blue.

It could - but Episode 36 of the anime is “The Worst Man in the Eastern Seas! Fishman Pirate Arlong!”, and chapter 96 is the “Meanest Man in the East!” referring to Luffy’s new Bounty post-Arlong Park.
