I think we are going to see some re-federation on a case by case basis
I've also seen people mention a "block by default" list on each instance, where some instances are "blocked by default" for new users, but people can remove them from the block list after a warning. There could also be a toggle to enable whatever the instance has set by default, so that admins can add new instances to that list and users don't need to do anything.
That way we don't need to discuss as much for the stuff that is annoying, but not necessarily bad (ex. bot instances, NSFW instances, etc.).
It doesn't exist yet, but that could be a future feature to work on
